Single Minded.  The flayed one attacks the last creature to hurt it. Stench. Any creature that starts its turn within 5 feet of the flayed one must succeed on a CON saving throw or be poisoned until the start of its next turn. On a successful saving throw, the creature is immune to this Stench for 24 hours.

Actions

Multiattack. The flayed one makes two attacks: one with its bite and one with its claws.   Bite. Melee Weapon Attack: 1d20+8 to hit, reach 5 ft., one creature. Hit: 11 ( 2d8+2 ) slashing damage plus 5 ( 1d10 ) necrotic damage. If the target is a living creature that is already paralysed, stunned, or unconscious, the bite deals an extra 2d6 damage.   Claws. Melee Weapon Attack: 1d20+8 to hit, reach 5 ft., one target. Hit: 12 ( 2d6+5 ) slashing damage. If the target is a creature other than an undead, it must succeed on a CON saving throw or be paralyzed for 1 minute. The target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

Reactions

Dead Blood. If an attacker scores a critical hit or an attack does 25 points of damage against the flayed one, it (or a target within 10 feet) is sprayed by radioactive sludge causing the attacker to take 4d10+7 poison damage on a failed DEX saving throw, or half on a success. This damage is radiation and reduces poison immunity and resistance by 1 step. This also causes the target to roll their radiation die to avoid gaining a mutation.

  Flayed Ones are twisted and ghoulish cyborg terrors afflicted by an ancient infection that breeds a hunger for organic flesh in them. Flayed Ones are constantly on the hunt for more flesh to wear. They are detested by both machine and men.
